Are you meaning should you tell your family about the pregnancy or tell them about the pain? I'd say definitely yes to telling them about the pregnancy if you haven't already. And pain in your back and shoulders has nothing to do with pregnancy at 3 weeks that I know of.

So you can't be 3 weeks pregnant, unless its your embryo that's 3 weeks old then in actual fact you'd be 5 weeks pregnant.
