I don't know unless she had missed periods (irregular periods) to start with or other medical problems that might be similar to pregnancy. I knew someone who was pregnant with twins and didn't know it-and she and her husband already had one child, a little girl! She had irregular periods, so when she missed her period, she didn't think anything about it. And then she was on some kind of medication for some type of medical problem (her thyroid, I think) anyway, she started hurting really bad and went to the ER and thought it was a kidney infection until she went to the bathroom and delivered the baby girl and she was stillborn; they then rushed her over to L&D and realized there was another one, a baby boy, who made it okay after being in NICU for a little while. That has been about 4 years ago, and he's doing well today! I still don't understand how she didn't feel twins moving around, though, especially after having one child already. But for whatever reason, she didn't know.
